Try using PC/Intel partition table instead of EFI GPT and rewrite the partition table.
The primary bootable partition should probably be [System].

Fat32formatter Messed Up Partitions

I was trying to format a 64gb SD card to FAT32 file system using a free program called Fat32formatter and when I pressed the "Initialize" button Windows went to a BSOD (Win8.1), shut down and upon reboot advised I no longer had an OS.
