I need help with fixing flash drive errors

I have 8GB usb flash drive formated in NTFS. If i plug the flash drive in usb port of my laptop Window 7 sometimes can't detect file system and show RAW as file system and show message that i have to format the drive if i want to use it. The flash drive is installation usb for Window XP which so far worked without problems.

I haven't used Testdisk so far but i have read some tutorials and i tried to fix the usb drive with it. I am using the Testdisk under windows 7 and when i analyze the drive i see this error in the screenshot: "Bad ending cylinder <CHS and LBA don't match>"

I used the option to write a new partition table but after this i still see this warning when i analyze the flash drive. Can someone tell me what this error mean and how to fix it?
