I can not get pass the above screen

Try to clone the disk using gnu ddrescue http://www.cgsecurity.org/wiki/Damaged_Hard_Disk and try again TestDisk on the clone.

Note that if the disk head is damaged (does your disk make some unusual noise ?), you won't be able to clone it.
