Photorec works on sectors of a disk or a partition and does not take into acccount any metadata in a file system related to files and folder organisation.
This is perfectly fine as this mode of operation is the only possible one for a tool of last resort.
Photorec sometimes finds metadata type information within a file header, for instance length information, though, and is processing it.
File undelete with TestDisk
